Explosives safety alert no. 92 | 03 August 2016 | Version 1

Due to the unavailability of explosives boosters from the regular supplier, a consignment of boosters was sourced from an alternate supplier. The new boosters had been used on site for overburden shots without incident.

However, when used in a pre-split blast initiated with 5 gram/metre detonating cord, boosters were found ejected from their holes during the post-blast inspection and identified to be cracked internally, but had not initiated. The incident investigation determined the newly-sourced boosters required 10 gram/metre detonating cord for correct initiation. (Source: QLD Govt – Department of Natural Resources and Mines)
